Should you have coverage with a dental and/or extended benefits insurance plan, the dental office will supply you with a claim form which outlines the total fee. Patients are responsible to pay the fees as they are due and submit the receipt to their insurance plan directly for reimbursement. We cannot guarantee eligibility and the amount of coverage varies with each plan.
